Notice of Privacy Practices and HIPAA

This notice describes how medical information about you may be used and disclosed, and how you can access it. Please review it carefully. The Laserie Medical S.C. uses, protects, and may disclose your protected health information (PHI) under HIPAA, applicable federal law, and Illinois law. You may request a full copy of the current Notice at any time.

Our privacy responsibilities

We maintain the privacy and security of your protected health information, give you notice of our legal duties and privacy practices, follow the terms of the Notice currently in effect, and notify you when required by law if a breach compromises your information. Where Illinois law provides greater protection than HIPAA, we follow Illinois law.

How we may use and disclose your information

• Treatment: providing, coordinating, and managing your care, and communicating with health care professionals involved in your treatment.
• Payment: billing, collecting payment, processing applicable claims, and related payment activities.
• Health care operations: quality review, staff training, compliance, auditing, licensing, and business administration.

We may also use or disclose information for other purposes permitted or required by law, as described in the full Notice.

Uses requiring your authorization

Certain uses require your separate written authorization, including certain marketing activities, sale of protected health information, use of clinical photographs or testimonials for advertising or social media, and other uses not otherwise permitted by law. You may generally revoke an authorization in writing for future uses.

Specially protected information

Some health information receives additional protection under federal or Illinois law, including HIV and AIDS information, mental health and developmental disability records, genetic testing information, and substance use disorder treatment records. When specific written authorization is required, we obtain it unless disclosure is otherwise permitted or required by law.

Your privacy rights

Subject to applicable law, you may inspect or obtain copies of certain records, request amendments, request confidential communications, request restrictions on certain uses, request an accounting of certain disclosures, obtain a copy of the current Notice, designate a personal representative, and file a privacy complaint without retaliation.

This notice does not authorize the use of your photographs, videos, testimonials, or protected health information for marketing or promotional purposes. Any optional authorization is addressed separately.
